After your player has accepted a spot on a CCFC team, you will need to complete the forms below.  The forms can be emailed to admin@capitalcityfutbolclub.com or brought to our paperwork drop-off (mid-June).
 

1. Copy of Birth Certificate
2. Medical Release Form - must be notarized
3. USYSA Membership Form - note:  3 signatures are required on this document
4. MOSSL Code of Conduct Form
5. 1 inch by 1 inch color photos of player (head shot) - please provide 2 pictures
6. Uniform Order Form
